DIRECTOR OF CARE COORDINATION
Sisters of Divine Providence, founders of LaRoche University, are currently accepting applications for a Director of Care Coordination . This position combines nursing knowledge with administrative skills and is responsible for maintaining a high standard of resident centered care. This position will be accountable for all functions related to the delivery of care, with special emphasis on creating and maintaining a homelike atmosphere. The mission of St. Joseph's Center , the Congregation's skilled nursing division, is to provide compassionate, quality, individualized care to the religious members who reside there. Essential Duties/Responsibilities: Affirm, foster, and demonstrate a commitment to the vision, mission, value, and philosophy of the Sisters of Divine Providence and promote it in a positive manner in all interactions and behaviors. Collaborate/communicate with the Provincial Council and the Marie de la Roche Administrator in matters related to the health of the Sisters in residence at St. Joseph Center or Providence Heights or those being considered for admission to St. Joseph Center. Ensures appropriate health, social and spiritual services are provided for residents and supports the philosophy of Patient Centered Care. Manage the program of care in St. Joseph Center and Providence Heights. Act as a Liaison with Life Pittsburgh and other outside health programs/services. Develop and implement individual care plans for all residents. Manage clinical staff inclusive of conduct expectations, roles and responsibilities, scheduling, staffing, and budgeting. Assess and provide on-going and targeted continuing education for clinical and support staff. Evaluate and provide counsel/resolution to resident care issues. Education/Qualifications: Graduate of an approved School of Nursing. Current and unencumbered licensure as a Registered Nurse in the Commonwealth of Pennsylvania. At least 3 years of experience as a supervisor in a long-term care facility or other related health care facility. Must have previous long-term care experience.
